Pre-registration is requested for this in-person seminar on Saturday, June 6 from 9 am to 12 noon in Bloomfield, CT. Please submit this form by end of day on Tuesday, June 2 so we can have a headcount for the light meal. The $50 fee is payable at the door by cash or check payable to Take Charge Foundation.
